Scheduling appointments
Our services are available to anyone with sport or exercise-related injuries or inquiries. Please note we are a scent free facility.
To schedule an appointment, please call the Clinic at 416-978-4678.
All new patients are required to complete a Registration Form and Health History Form. In order to speed up the registration process please download and complete these forms prior to arrival for your first visit. Returning patients need only update information that is out-of-date.
All information gathered for treatment is confidential except as required or allowed by law or to facilitate diagnosis (assessment) or treatment. You will be asked to provide written authorization for release of any information.
Private patients (i.e. patients who are not U of T students or carded athletes) require a referral from a family physician in order to schedule an appointment with a sport physician. To schedule an appointment with a therapist, a direct referral from an external physician is acceptable. The clinic fax number is 416-971-2846.
Please note that the clinic does not treat patients with workplace or motor vehicles injuries and we reserve the right to cease treatment as determined to be appropriate.
Cancellation policy
We have a strict cancellation policy outlined in our fees, and posted in the clinic. We require 24 hours notice (during business hours) for cancellation without penalty. Please note we have a cancellation mailbox available 24 hours.
